Beyond the Needles: Comparing Neuromuscular Rehabilitation to Hardware & Software in the Body

During a recent clinical session, a patient asked a question often heard over the last 15 years: “What is acupuncture actually doing? How does it work?” 


It’s a great question. Acupuncture can kind of look like a mystery. In Traditional Chinese Medicine (TCM) circles, acupuncturists talk about the flow of Qi and balancing meridians. But we can also frame this through Integrative Medicine with a more contemporary nueromuscular and structural framework, talking about physical components of the body and the physiology that frames these components. The body can be understood in a context of "hardware" vs "software" in relation to pain and other symptoms; and integrating treatment to address both hardware issues as well as software can lead to some phenomenal results.

When we use Orthopedic or Sports Acupuncture, we aren't just treating a symptom. The approach is to address both the hardware, as well as treat deficiencies in the software at the exact same time. Here is a little insight into how it works.


1. Fixing the "Hardware" (The Infrastructure of the Body)

Your body’s hardware would consists of its physical, structural pieces: muscles, ligaments, tendons, fascia, joints, and bones.


When you experience a structural issue—such as a dense muscle knot (myofascial trigger point), a strained ligament, or tissue degeneration from chronic poor posture—the local tissue changes. It becomes ischemic (starved of blood flow), tight, and choked off from fresh nutrients.


We use acupuncture needles to physically alter and repair this hardware:

  • Local Tissue: Inserting a sterile needle creates an awareness in the body that leads to changes at the site of structural dysfunction. You could liken it to a biological flare gun which alerts and brings on board your body's immune system to a localized issue where protein synthesis and repair is needed

  • Vasodilation and Blood Flow: Research shows that needle stimulation triggers a massive release of vasodilators like Nitric Oxide (NO) and Calcitonin Gene-Related Peptide (CGRP). This causes blood vessels to rapidly dilate, increasing local blood circulation and flooding the injured tissue with fresh oxygen and nutrients.  

  • Cellular Repair: This influx of blood brings vital white and red blood cells, triggering a localized inflammatory cascade that accelerates cellular turnover, clears metabolic waste, and physically repairs tissue.


2. Addressing the "Software" (The Neurological Command Center)

Even if your physical structures are intact, they won't function correctly if the command signals are corrupted. This is where your software comes in: your Nervous System (the Central Nervous System consisting of your brain and spinal cord, and the Peripheral Nervous System running through your limbs).


Your nervous system sends the electrical impulses that dictate when a muscle should contract, relax, stabilize, or fire. However, due to stress, acute trauma, or chronic pain, your software can experience a "glitch."


In sports medicine, we call this neuromuscular inhibition. When a joint or tissue is threatened, the nervous system adapts by altering the motor output. A muscle might get stuck permanently "locked on" (hyperactivity), or it might get completely "blown out," refusing to fire when you need it to (inhibition).


Acupuncture acts as a system reboot, getting the electrical impulses of the nervous system on line and firing by utilizing Muscle Motor Points and Electroacupuncture:

  • Targeting the Neuromuscular Junction: A muscle motor point is the precise anatomical location where a motor nerve enters the muscle belly. Studies show that these points have the lowest electrical resistance on the skin and are densely packed with nerve endings.

  • Speaking the Language of the Body: By placing needles precisely into these motor points and introducing a gentle electrical current (electroacupuncture), there is a bridging in the body through the language of your nervous system.

  • Resetting the Circuit: The electrical impulse travels along the peripheral nerves to the spinal cord, likely interrupting a signal-loss along the neural pathway. It creates the space for a re-set: a hyperactive and locked-up nerve can calm down and downregulate, while simultaneously telling a weak, uncooperative connection to "wake up" and restore its proper motor control loop.  


3. Why the Synergy Matters: Physical + Electrical

Many standard therapies only address one side of the coin. If you only fix the hardware - for instance, by getting a heavy deep-tissue massage or foam rolling a tight muscle -you might feel temporary relief. But if the underlying software signal (the nerve) is still telling that muscle to lock up for protection, the knot will return within hours. Or if the software is not connected effectively to the structure of the body pain and dysfunction may continue as the body tries to compensate through less efficient patterns.


By integrating a functional, neuro-anatomically based acupuncture approach, Dr Tyler Andres, DAOM, LAc works to create a whole-spectrum intervention. In that intervention he is looking to evaluate and address both issues with the body, through the hardware, as well as the nervous system, through the software.

Studies demonstrate significant increases in local tissue perfusion and oxygenation following intramuscular needling due to the release of CGRP and Nitric Oxide.

  • On Autonomic Nervous System Regulation: Frontiers in Neuroscience (2025). Meta-analyses of clinical trials demonstrate that acupuncture significantly modulates Heart Rate Variability (HRV), shifting the body away from sympathetic "fight-or-flight" overdrive and into a restorative, parasympathetic state.
